Our referral process is quick and easy. Most referrals are from GPs, school nurses and teachers.

Are you a professional wanting to refer a child to us?
There are three ways you can refer a family to MEND.

  • You can either complete our online referral form

  • Or if you’d rather complete a hardcopy referral form and post it to us, you can download a printable registration form

  • You can also refer a family by calling us on our free phone number 0800 2300 263

What MUST happen before a child is referred to MEND?
Before making a referral, you must talk to the child’s parent or main carer to:

 

  • Discuss the MEND programme you want to refer them to

  • Make sure they agree to their child being referred to MEND MEND

  • Tell them that MEND will contact them by phone to talk through the next steps

If a referral is made by a professional other than a GP, we will write to the child’s GP to tell them that the child has been referred to us.

Can parents or carers refer their own children to MEND?
Yes. Parents or carers can register their child’s details with us either on our Join a programme or by calling 0800 2300 263 for free.
